The following forms are required for participation in varsity athletics at Mitchell College. All forms (excluding those completed online) should be mailed to:

1st & 3rd Years of Participation at Mitchell College - Includes All Transfers

* All Medical Clearance Forms:

1. Physical Examination Form - download, print, and have a physician complete

2. Clearance Form - download, print, and have a physician complete

3. Athlete Information Form - complete online

4. History Form - complete online

* Assumption of Athletic Risk Release - complete online

* Student-Athlete Biography Form - complete online

* Copy (front & back) of insurance card

2nd & 4th Years of Participation at Mitchell College
